Announcement
UCP of the Golden Gate will award two grants worth up to $4,000 each for the purchase of any device or equipment that will help a person (child or adult) with disabilities attain greater independence, improve access, or in some way enhance their quality of life.
Eligibility
Any person (child or adult) with a physical or cognitive disability who resides in (Marin, San Francisco, Alameda, or Contra Costa) is eligible to receive this award.
Application
Applications for this award are available on line at www.ucpgg.org. Completed applications can be mailed to the UCPGG offices at 1970 Broadway Suite 115, Oakland CA 94612 or faxed to 510-839-1329.
Applications must be received by 4:30 p.m. on Thursday June 30, 2011. Winners will be announced on Friday July 8, 2011.
